How to Apply for June & September 2024 & January 2025 Classes 

Step 1

COMPLETE YOUR DICAS APPLICATION.

ACCEPTANCE decisions are rolling and ongoing. Applicants whose applications are COMPLETE will be notified within 24 hours of RECEIPT.

APPLICATIONS WHICH ARE MISSING ONE OR TWO ITEMS WILL STILL BE CONSIDERED.

There are 4 Quadrants in the DICAS application. Be sure to complete all four.

We advise that you not wait until all sections of your application are complete to submit it.

BE SURE TO COMPLETE THE PRIORITY NUTRITION CARE QUESTIONS UNDER PROGRAM MATERIALS.



Step 2

SUBMIT ROTATION SCHEDULE FORM 

JUNE & SEPTEMBER 2024 CLASSES: SUBMIT THIS FORM WITH AT LEAST YOUR FIRST ROTATION PRECEPTOR BY MARCH 30, 2024. 

COMPLETE AS MANY SECTIONS AS YOU ARE ABLE. SAVE THE EDIT LINK FROM THE EMAIL YOU RECEIVE WHEN YOU SUBMIT THE FORM AND CONTINUE TO ADD SITES AS YOU SECURE THEM.  

This form is used to list the scheduling of Rotations and locations for your rotations and preceptors. 
Step 4

HAVE YOUR ROTATION PRECEPTORS COMPLETE CONFIRMATION FORMS.

CLASS STARTING IN JUNE OR SEPTEMBER 2024 - AT LEAST FIRST ROTATION PRECEPTOR CONFIRMED BY MARCH 30, 2024.

HAVE YOUR ROTATION PRECEPTORS COMPLETE THE FORM 

Your Preceptor fills out this form which is located on our website www.PNCDDI.com on the Preceptor page.(LINK BELOW) MAKE SURE THAT YOU PROVIDE YOUR PROPOSED PRECEPTOR WITH YOUR EMAIL ADDRESS and you will receive an email when the form is submitted.  The information on these forms is used to confirm that the Preceptors you contacted have agreed to precept you.  This is not an application to be a preceptor. They must be completed and submitted by your PRECEPTORS, not BY YOU, the APPLICANT.  They may use the Preceptor Contact Form button below, and the form is submitted electronically.  There is additional information using the Preceptor Information button below. As an applicant, you would benefit from becoming familiar with this information.
