TO START IN SEPTEMBER 2026 

1. Applicants who submit a complete application will be notified within 24 – 48 hours of their offer of admission and must notify the Internship of their decision and pay the nonrefundable $2500 deposit within 24 hours of receipt of their offer of admission. 

Payment of the nonrefundable deposit  will trigger the initiation of the onboarding process, which includes but is not limited to: 

      *vetting and approval of preceptors and sites,

      *initiation of affiliation agreements and contracts with your sites not already in place 

       *compilation of medical information such as immunizations and immunity 

        *professional liability insurance requirements 

         *background checks and drug screens

Your application will be acted on with or without Preceptors identified. However, a Rotation Schedule form must be submitted with the FIRST ROTATION PRECEPTOR identified within 30 days of accepting admission to the Internship.

COMPLETE AS MANY SECTIONS AS YOU ARE ABLE. SAVE THE EDIT LINK FROM THE EMAIL YOU RECEIVE WHEN YOU SUBMIT THE ROTATION SCHEDULE FORM AND CONTINUE TO ADD SITES AS YOU SECURE THEM.
